Knee pain is the most common complaint Anthony hears from riders, and Phil Burt — who spent years as Head of Physiotherapy at British Cycling — has a direct diagnostic: where does it hurt? The location of the pain is the diagnostic. Front of the knee means the quad-patellar system is overloaded, which points at a saddle too low, too far forward, or cranks too long. Back of the knee means the hamstrings are overextending, which is a saddle too high or too far back. Sides of the knee point at rotational issues — cleats that force the foot into an unnatural angle.

The trap most riders fall into is ignoring it, adjusting something randomly, or throwing money at orthotics and straps before checking the three basic variables: saddle height, saddle fore-aft, and cleat alignment. In Phil Burt's experience, those three resolve roughly 80% of cycling knee complaints without any further intervention.

If you adjust all three and pain persists beyond two weeks, it is time for a professional bike fit and potentially a physiotherapy assessment. Some knee issues are truly structural — prior injury, patellar tracking problems, or ITB tightness — but you should rule out the mechanical causes first because they are the cheapest and fastest to fix.

    The location of cycling knee pain is almost always diagnostic. Anterior pain points at saddle too low or forward, posterior at saddle too high or back, lateral at cleat rotation. Fix the three basic fit variables before doing anything else — they resolve the majority of cases.

  1. Map where the pain sits

    Is it front, back, inside or outside of the knee? This narrows the cause before you touch anything. Write it down so you can track whether adjustments change the location or intensity.

  2. Check saddle height

    For anterior pain: raise the saddle 3-5 mm. For posterior pain: lower it 3-5 mm. Use the heel-on-pedal check: at 6 o'clock with your heel on the pedal, your leg should be almost straight with no hip rocking.

  3. Check cleat rotation

    For lateral or medial pain: look at your cleat alignment. Your foot should sit at its natural angle on the pedal — not forced inward or outward. Cleats with float (6-9 degrees) reduce the risk of rotational stress.

  4. Check saddle fore-aft

    For anterior pain: slide the saddle back 5 mm. For posterior pain: slide it forward 5 mm. Use the plumb-line check from the kneecap to the pedal axle as a starting reference.

Why does my knee hurt only after an hour of cycling?
A mild fit issue is tolerable at low load but accumulates stress over time. After 60-90 minutes the tissue reaches its tolerance threshold and pain appears. The fact that it is time-dependent rather than intensity-dependent strongly suggests a mechanical cause.
Can cycling damage your knees?
Cycling is one of the lowest-impact sports for knee joints. On a properly fitted bike, it is protective rather than destructive. Knee damage from cycling almost always comes from a position problem, not from the activity itself.
Should I stop riding if my knee hurts?
Reduce intensity and duration, and adjust the suspected variable. If the pain is sharp, worsening, or does not respond to fit adjustments within two weeks, stop and see a professional.
Can crank length cause knee pain?
Yes. Cranks that are too long increase the knee flexion angle at the top of the pedal stroke, which can overload the patellar tendon. Shorter cranks (165-170 mm instead of 172.5-175 mm) are increasingly recommended for riders with anterior knee issues.
Does cadence affect cycling knee pain?
Higher cadence (85-95 RPM) reduces the force per pedal stroke, which lowers stress on the knee joint. Riders with knee pain who habitually grind at 60-70 RPM often improve by shifting up their cadence.
When should I see a physiotherapist for cycling knee pain?
If pain persists after two weeks of fit adjustments, or if the pain is sharp, swelling is present, or the knee locks or gives way. At that point the issue may be structural rather than mechanical and needs clinical assessment.
